DoxyPEP: A “Morning After” Pill For STIs Could Be On The Horizon

Preventing the spread of some sexually transmitted infections (STIs) could be as simple as taking a couple of pills. At least‚ this is a possible scenario if DoxyPEP becomes more widely available worldwide. The treatment uses an antibiotic as a post-exposure prophylaxis – a preventative treatment that can stop the spread of bacterial infection.Research on this approach is limited‚ but it has shown early positive results. So much so‚ in fact‚ that the US Centers for Disease Control and Prevention proposed guidelines for its use in October 2023‚ although these are yet to be finalized.“DoxyPEP is what people commonly call it‚ but 'doxycycline post-exposure prophylaxis' in full is when people take 200 milligrams of doxycycline‚ which is an antibiotic‚ and they take it usually within 24 hours‚ but up to 72 hours of condomless sex‚” Dr Manik Kohli‚ a Clinical Research Fellow at the UCL Institute for Global Health‚ told IFLScience. “This has been shown in three clinical trials to be effective in gay and bisexual men and in transgender women at preventing chlamydia‚ syphilis‚ and gonorrhea as well.”Scientists see no reason why those results wouldn’t apply to everyone‚ but medical interventions require rigorous trials and so far those trials have included just a small section of people. “We still don't know enough about DoxyPEP efficacy in cisgender women‚ trans men‚ and non-binary people with vaginas. There's only one existing study in cisgender women in Kenya‚ and the data from that was sort of mixed – it showed that DoxyPEP didn't work as well. But there are also some suggestions that that was because the women in the study weren't taking DoxyPEP as regularly as other people in other studies‚” Dr Ben Weil‚ head of research and community knowledge generation at The Love Tank‚ told IFLScience.More clinical trials are being conducted around the world and preliminary results suggest a reduction in the incidence of chlamydia and syphilis by over 80 percent. But the most interesting interim results come from real-world data from the San Francisco AIDS Foundation’s Magnet sexual health clinic. Dr Hyman Scott‚ who presented the results at the recent Conference on Retrovirus and Opportunistic Infections‚ reports how the clinic offered DoxyPEP to everyone who was already taking PrEP (pre-exposure prophylaxis) against HIV.“We offered DoxyPEP to all those folks. We wanted to evaluate the impact of this intervention after we rolled it out in our clinic‚” Dr Scott explained to IFLScience‚ “and we saw that the impact was rapid and it was significant.”A small dose of this antibiotic might be a way to prevent STIs.Image credit: Cliff Day/Shutterstock.comThere are now over 2‚000 people who are currently using DoxyPEP for STI prevention and the uptake is generally equitable across race‚ ethnicity‚ and gender identities‚ although fewer trans men and no cisgender women were taking it at the time of writing. This is related to the lack of clinical trials including them. The results‚ though‚ are extremely promising.“We saw a 58 percent reduction in STI incidents in less than a year after implementing this‚” Dr Scott told IFLScience. “That's the big highlight for what we saw in this analysis‚ and that number was a reduction in any STI. It was most dramatic for syphilis‚ where we saw a 78 percent reduction and we saw a 67 percent reduction for chlamydia.”The reduction in gonorrhea cases was more modest (11 percent) but it might be possible that the downward trend is not as rapid as with the other two diseases. All of this is extremely encouraging‚ but researchers hope for more studies that can provide further insight into the effectiveness across different populations and at different doses and regimens.“The research that exists does show that DoxyPEP is efficacious at preventing bacterial STIs‚ but the evidence is limited. I mean‚ there haven't been that many studies‚ and there aren't that many studies that have looked at more flexible dosing regimens‚” Dr Weil told IFLScience.“Just like with HIV prep or any kind of contraception‚ we know that the more options there are available in the ways of taking an intervention‚ the better it is because it can meet people who have flexible lifestyles and want to take things very‚ very differently.”Another area of interest is if the use of doxycycline becomes more widespread‚ could that cause antibiotic resistance? Some preliminary studies are leaning towards no‚ and suggest it definitely won’t impact bacterial STIs‚ but there are questions to be asked about the bacterial colonies that live within us and on us.“That is something that we definitely want to [and] need to understand more about. I don't think that means that we should delay implementing DoxyPEP‚" Dr Kohli told IFLScience. "DoxyPEP is evidence-based and it is efficacious. We're currently seeing STI rates rising across the board and we're seeing congenital syphilis rising for the first time in decades. So there is a real need to actually address that.” Although the evidence so far looks promising‚ it's important to note that even if DoxyPEP is implemented tomorrow‚ STI prevention is not just a matter of drugs. Stigma‚ inequalities‚ sexual and racial discrimination‚ limited funding for sexual health clinics‚ and the availability of interventions and education for different groups all make it more difficult to stop the spread of these diseases‚ which harms individuals and society as a whole.

New “Starry Night” Gecko Species Found In India Named After Van Gogh

“Life imitates art” might be an overused cliché‚ but a newly discovered gecko species found among rocks in the Western Ghats in India appears to be taking the phrase quite literally.Back in April 2022‚ a team of researchers had set out on an expedition in the deciduous forests of the southern Western Ghats – an ecoregion in the south of India – on the hunt for members of Cnemaspis‚ a genus of geckos that are active during the day.During their search‚ they came across a small lizard with a “striking colouration” on its back‚ made up of small blue dots‚ that they found to be reminiscent of Starry Night‚ one of Post-Impressionist painter Vincent van Gogh’s most famous works. When the team discovered that the gecko they had found was a brand new species‚ they decided to name it after the painter‚ dubbing the little lizard Cnemaspis vangoghi. It was an impressive find to make‚ as the newly named gecko only measures around 3-4 centimeters (around 1-1.5 inches) in length.This photo has strong "just a lil guy" energy.Image credit: Akshay KhandekarIt now joins an elite gang of van Gogh-inspired "Starry Night" creatures‚ alongside the Starry night harlequin toad and the Starry night peacock spider.The researchers also identified another new gecko species‚ Cnemaspis sathuragiriensis – named after the hills it was found on – though both new species share some interesting characteristics.“The two new species are distributed in low elevation (250–400 m asl.)‚ deciduous forests of Srivilliputhur‚ and add to the five previously known endemic vertebrates from Srivilliputhur-Megamalai Tiger Reserve‚ Tamil Nadu‚ India‚” explained study author Ishan Agarwal in a statement.Both geckos were also mostly active during the early morning and evening and were primarily found on rocks – but they were also only found in a limited range. Specimens of C. vangoghi‚ for example‚ were only found in two areas within 15 kilometers (9 miles) of each other‚ which Agarwhal called “an interesting case of micro-endemism in low-elevation species.”The discovery was made during a four-year survey that formed part of a wider project exploring the lizards of Tamil Nadu‚ a state well known not just for its cultural heritage (it’s home to four World Heritage Sites)‚ but its rich array of flora and fauna.“Tamil Nadu is an exceptionally biodiverse state‚” said Agarwal. “We expect to name well over 50 new species of lizards by the time we are done [with our expeditions]!” With the two new species of gecko described in the study‚ the team is already off to a good start – although making cool scientific discoveries doesn’t come without its downsides‚ apparently.“I also had more than 500 tick bites during that summer trip‚ with the highest densities in the low-elevation‚ dry forests of Srivilliputhur‚ where the new species are found‚” Argwhal recalled.Some might argue the itchiness and risk of disease were worth it to find a gecko resembling an iconic artwork.The study is published in ZooKeys.

Tapeworms Have Been Upsetting Stomachs For At Least 99 Million Years‚ First Fossil Suggests

An “exceptional” 99-million-year-old tentacle trapped in amber is the first partial body fossil of a tapeworm ever discovered‚ suggesting the parasites have been wreaking havoc on intestines since at least the mid-Cretaceous.Despite being found in nearly all marine‚ freshwater‚ and terrestrial ecosystems‚ tapeworms (Cestoda) are rarely preserved in the geological record. In fact‚ the only widely accepted example before the Quaternary are eggs discovered in fossilized shark poop from the Permian."The fossil record of tapeworms is extremely sparse due to their soft tissues and endoparasitic habitats‚ which greatly hampers our understanding of their early evolution‚" Bo Wang‚ the lead researcher of a study describing the latest discovery‚ said in a statement. But now‚ his team have "reported the first body fossil of a tapeworm‚" he added.Not only that‚ but the find is also arguably the most convincing body fossil of a flatworm ever discovered.The fossilized tapeworm (A/B)‚ compared to a modern tapeworm (C).Image credit: NIGPASThe prehistoric parasite‚ preserved in mid-Cretaceous Kachin amber from northern Myanmar‚ bears resemblance to tapeworms that are around today and infect elasmobranch fish like rays and sharks. The researchers‚ therefore‚ think that this little critter likely did the same‚ which begs the question: how did a marine parasite end up caught in amber? While they don’t know for sure‚ the team does propose one possible explanation. Tapeworms are what’s known as endoparasites‚ meaning they live inside their hosts. Trypanorhynchs – the group to which the newly discovered worm belongs – are intestinal interlopers‚ clinging to their hosts' guts and absorbing nutrients. As the specimen was found in a near-shore environment‚ it could be assumed that it was lurking in the bowels of an elasmobranch that was stranded by a tide or storm.The unfortunate fish may then have been scavenged upon by a land-based predator – perhaps a dinosaur if the paleoart is anything to go by – which ripped its tentacle free‚ and allowed it to get trapped in resin.It’s a purely speculative scenario‚ the study authors note: “the truth may be far beyond our imagination.”“No matter how unlikely the preservation of this tapeworm in amber‚ our study highlights that amber has the potential to capture unexpected life details in deep time‚” the authors conclude‚ referencing their discovery that resin is capable of preserving the internal structures of parasitic worms too.“Our study not only provides a remarkable example of a marine endoparasite preserved in amber‚ but also highlights the importance of amber research in paleoparasitology.”The study is published in the journal Geology.

MSNBC’s Joy Reid and her favorite Republican‚ Michael Steele‚ oohed and awed on Thursday’s The ReidOut over President Joe Biden’s fundraiser with former Presidents Bill Clinton and Barack Obama and a cast of celebrities‚ including CBS’s Stephen Colbert. Not only did they fawn over Biden‚ they thought the fact that Donald Trump “can’t do this” and that George W. Bush won’t do it for him deserved to be some election-defining revelation. Steele was joined by Columbia University journalism dean‚ Jelani Cobb‚ whom Reid referenced when she gushed‚ “But I think‚ to Jelani’s point‚ it's just the fact that Joe Biden can gather former presidents to himself. The fact that he can gather a celebrity presence to himself. These are things that whatever else Donald Trump can do‚ he can gather Marjorie Taylor Greene to himself‚ he can't do this.”     Steele agreed‚ "Yeah‚ it's‚ you know‚ I'm sitting here looking at this‚ and‚ you know‚ realizing‚ we've got one former living president in Donald Trump. That's what we got and you sit there and say‚ ‘hmm‚ what do you do with that?’” Most people would say nothing because liberal celebrities endorsing liberal presidents isn’t exactly earth-shattering news‚ but Michael Steele is not most people‚ “George Bush is not coming to an event for Donald Trump. George Bush is not going to get out there and bring a celebrity cadre and donors to Donald Trump and that speaks volumes about this moment for the Republican Party. When the former president who led this party is not a factor on behalf of Donald Trump in his effort to regain the White House.” Put Trump aside‚ Bush couldn’t get celebrities to come out for anyone he would support because‚ again‚ celebrities‚ especially the ones MSNBC likes‚ are overwhelmingly liberal. Celebrities‚ like Colbert‚ weren’t exactly big fans of him either. Our republic is under siege by an aspiring apparatchik class

America is developing a governing class that doesn't reflect American culture. But let's start telling that story by crossing an ocean. For 75 years‚ the Ecole Nationale d’Administration was an elite pathway in France‚ recruiting at the socioeconomic top and credentialing for the sociopolitical top. It was the place that purportedly drew the “crème de la crème‚” the school that the BBC described a few years ago as the destiny for young adults that was “coveted above all others.” The high status of a school of administration is instantly revealing‚ speaking of a society that elevates its bureaucrats to elite status. Imagine the pride of French parents‚ knowing that their son was being groomed for a position as a deputy assistant minister of culture.The growing rigidity and professional isolation of the political class is a new development in American culture and not a healthy one. Elite bureaucrats are a symbol of a culture that isn’t primarily about making and doing‚ and it’s a sign of a society that values leisure and paid vacation above creativity and risk. It’s the “neutral” gear on the national stick shift. Meanwhile‚ strangely enough‚ the University of Wisconsin is reporting surprising levels of interest in its School of Public Affairs and is now crafting a new undergraduate program in government studies to meet the demand: “There are almost 500 students taking public policy certificates at UW-Madison and it’s one of the fastest growing programs on campus‚ according to Webb Yackee.”We have 19-year-old Americans who aspire to become government apparatchiks. It’s one of the fastest growing programs on campus. From disqualifying to normalThis cultural shift reflects the new shape of a political class that lives and dies in politics‚ never brushing up against the private sector or a productive function. I watched the mayor of Baltimore speak in front of a collapsed bridge this week‚ and then I looked him up: He graduated from college‚ then went directly on to the staff of a city council member‚ then ran for city council‚ then ran for mayor. As I watched Mayor Brandon Scott‚ I saw an ad for a new candidate for governor in California‚ where I live. Betty Yee is running for the state government’s top job from her seat as the state controller‚ where she’s in charge of the $73 billion deficit. Clearly‚ then‚ she’s eligible for promotion.So I looked her up: Board of Equalization‚ State Lands Commission‚ state budget director. She’s a 66-year-old woman who has spent her entire adult life bouncing from government job to government job. Her campaign bio brags about her “nearly 40 years in public service.” That should probably be disqualifying‚ but now we just call it normal. A complete career pathNot so long ago and with lingering examples‚ our political norm was that politics was a second profession. The dentist Paul Gosar ran for a seat in the House of Representatives‚ where he serves alongside the emergency room doctor and U.S. Navy veteran Ronny Jackson. The lawyer and soldier Andrew Jackson ran for Congress. The retired army officer Dwight Eisenhower ran for president. City councils and school boards have usually been full of local small-business owners but are filling up with government employees who dream of a seat in the legislature.The second profession is becoming the only profession‚ a complete career path.There are counterexamples‚ of course. Lyndon Baines Johnson probably dreamed of public office in the womb. But the growing rigidity and professional isolation of the political class is a new development in American culture and not a healthy one. The other emerging path to public office is through the helping professions‚ like the social worker Katie Hobbs. In California‚ the legislative committee created to resolve the state's $73 billion deficit includes Assemblyman Corey Jackson‚ whose proudest life accomplishment is his doctorate in social work. Coming to politics from politics‚ or from grant-funded and government-dependent nonprofits‚ leading government after working for the government‚ elected officials run a country that they’ve never visited. George McGovern famously bought a hotel after he retired from politics and complained vociferously about the onerous regulations imposed on business by politicians. The better time to learn that lesson is before going to politics‚ and we're losing that path in a hurry.Do this yourself: Look up a few of your elected officials‚ starting with cities and counties and passing through the state government on your way to Congress. See how many of your representatives ever had a job of some kind. Then remember where they come from when you hear them speak about how to solve problems and make the world a better place.In France‚ by the way‚ the ENA is dead‚ replaced by French President (and ENA graduate) Emmanuel Macron with the Institut National du Service Public. The old ENA had a campus in Strasbourg‚ while the new and supposedly more democratic and diverse school has a campus in ... well‚ in Strasbourg.U.S. Army veterans will similarly recall the moment the Pentagon capitulated to activist demands to eliminate the School of the Americas‚ changing the sign in front of the building to say that the classrooms now house the Western Hemisphere Institute for Security Cooperation. It’s totally different‚ we swear.
